You must have Adobe Acrobat Reader installed to view and/or download these forms. Please use one of
these options to return the forms to Delaware Community Development Corporation:
1) EMAIL - Depending on which version of Adobe Acrobat Reader that is installed on
your computer, these forms may be filled out on your screen. Once the forms are complete, click
SAVE AS to download the completed forms to your hard drive (NOTE: remember where you save them for
easier retrieval when emailing).
You can then submit them as attachments via email to one of our loan officers.

2) FAX - If you do not have a current version of Adobe Acrobat Reader, the forms may not
display the fill-in fields. In this case, print the forms, complete them and fax to DCDC at 302-652-5679.
Remember to keep a copy of the completed form for your records.
